Simple spreadsheet- 2 columns adding into a 3rd column

Can somebody help me with the formula that will add 2 seperate columns into a 3rd?
 

Excel Facts

Move date out one month or year
Use =EDATE(A2,1) for one month later. Use EDATE(A2,12) for one year later.
In order to do this, we'll need to know a bit more information, i.e. what are your "rules" (where to break the columns)? Also, providing a few samples would be helpful.
