Sorry to seem stupid

Please help all I want to do is - I have a list of names in column A, and list of dates in row 1. I want to prompt the user with either an input box or a user form to first select a name, then select a date, and then using a third input box enter a vaule into the relevant cell. Surely it can't be too difficult, but it's driving me mad.
