I want to in put data to cell from a form. How could I design this form ?

Already, excel has form. please check Data(D) > Form(O).

If you want to use an userform, you can make it on VBE.
 1.Press [Alt] and [F11] key sametime.
 2.Press [Alt] and key sametime.
 3.Press key.
