If you add a helper column in E and add together the cells A-D (A2&B2&C2&D2)
Then (for ease) unmerge your New, Old and Blank cells and add each individually so you have New New Old Old Blank Blank (or if you want to keep these as they are add a helper row with these and hide it.
Do the same with the names in cell B16 downwards.
In cell D16 enter: =COUNTIF($E$2:$E$12,$C16&E$15&$D16&E$14)
fill across and down. The results match what you want.
