Code for Form to enter data in a different worksheet based on User name

Hello Everyone,

Can some please help me with a code to create a form that has multiple data entries.

There will be a master file where the form will be present and based on the user selected the remaining data should populate in the specific excel file of the user.

The headers are the same in each excel file. Here is what I want in the form.
User Name
Department
Year Enrolled
Subjects (This should have multiple options in the drop-down menu (Physics, Chemistry, Biology etc.)

So based on the username all the data including the user name should be populated in the user specific excel file.

Any help will be appreciated!

Excel Facts

Show numbers in thousands?
Use a custom number format of #,##0,K. Each comma after the final 0 will divide the displayed number by another thousand
