excel forms to data base

I want to create an excel form in excel
the first sheet will be the form and the second sheet will be the data Base,
I need to Generate and automated document after filling the form to save separately in another folder with a new name...

I need to show you guys the Excel worksheet but i don't know how to post it here

Excel Facts

Select a hidden cell
Somehide hide payroll data in column G? Press F5. Type G1. Enter. Look in formula bar while you arrow down through G.
