Formula to look up on both x and y axis

Hello,

I have a scoring matrix which I'm trying to use to assign a numerical value. I need a formula for Column D that will look up both on the x and y value. So, I want the results in Column D to automatically recalculate as updates to columns B and C are entered. Here's what it looks like:
