Vlookup with two columns?

Possible to do a v-lookup using "and" to look up both first and last names (currently in two columns)? Alternatively, suppose I could combine first and last... Other ideas?
 
noticed I placed "a" to "g" instead on Conchio1 to Conchio7. a to g is fine, used this so faster to type.

That would be:

Not CSE right? noticed the double index...
MATCH type set to false, why not zero, isn't it the same? Thanks to shed some lights.
