Countif Value is matched

Hi Everyone,

I want a formula in cell A4 that will match the value in C4 with the range C15:C5000 . If matched (in this case first value is cell C4 is matched with C15) then get that cell reference, add two rows and go to next column and check if next three cells are empty or filled (in below case these are D17:D19) and return the number of filled cells in cell A4.

Same for Cell A5 is required.

=COUNT(INDEX($D$15:$D$5010,MATCH(C4,$C$15:$C$5000,0)+2):INDEX($D$15:$D$5010,MATCH(C4,$C$15:$C$5000,0)+5))
