INDEX MATCH PROBLEM

Please can I get an Index-Match formula to find the corresponding day and date in this image for the average,maximum and minimum values in column J

The maximum formula is

The minimum formula is

And the average formula is

Please I just want an Index-Match to know its corresponding days and dates.
