Issue Moving data between worksheets using INDEX and Match it is offset 1 column

I have data I am trying to reference to two different items, a Task # and then the title of the position working. The data is being pulled but it is offset 1 column, it pulls the data correctly related to the task but it is offset by 1 column.

The formula I am using is =INDEX('Data-CT-SG'!$B$124:$W$126,MATCH($A4,'Data-CT-SG'!$B$124:$B$126,0),MATCH(D$2,'Data-CT-SG'!$9:$9,0)) and is in the highlighted area below.

As you can see the "2" is under the Project Principal INSTEAD of being under the QA/QC Manager. This is the case across all fields, being offset by one column? Any help would be appreciated in identifying this issue.

It's because you are using an entire row on the 2nd match, try
Excel Formula:
=INDEX('Data-CT-SG'!$B$124:$W$126,MATCH($A4,'Data-CT-SG'!$B$124:$B$126,0),MATCH(D$2,'Data-CT-SG'!$B$9:$W$9,0))

It's because you are using an entire row on the 2nd match, try
Excel Formula:
=INDEX('Data-CT-SG'!$B$124:$W$126,MATCH($A4,'Data-CT-SG'!$B$124:$B$126,0),MATCH(D$2,'Data-CT-SG'!$B$9:$W$9,0))
Perfect thanks so much I see that now my example was using the full set of data!
