How can I create a multidimensional array of integers

I need to create a multidimensional array for columns for each month. I use arrays all the time but it has been a long time since I used a multidimensional array.

The array needs to contain
1) The month's number, 1 through 12.
2) The number of columns for the month, this will be a 4 or 5 for the number of weeks in the month.
Months 3, 6, 9 and 12 will always have 5 weeks and the rest will have 4 weeks.
3) The column number that pertains to those weeks.

It is turf getting old, can someone spark my memory?
