ActiveX checkbox list form

Hi all.
I am new to this forum and try to programm an active checkboxlist. trying for several days now and i get stuck on getting the VBA working.
I have a list in excel with activex checkboxes in every row.

I want to see that when the checkbox(es) of certain row(s) is checked, that then the content of the previous cell and another cell will be copied to a another sheet(2) in a list form.
and of course when unchecked this will be removed from the sheet(2)

Please can you advise how i get this done.
