Button to Add row to table and Prompt for fields

Good morning,

I'd like to add a button to my spreadsheet that prompts me to fill in a number of fields from my table and then adds a row at the bottom of the table with the information entered. Could someone point me in the right direction? I've tried searching through the forums but couldn't find anything.
