Create a form for an event

I need to create a form that everyone can use to enter the entrants for a race with their past race times.
1. So I need the date of the event
2. Then I need the track where the race is at
3. the race number
4. name of entrant 1
past race info
name of entrant 2
past race info
name of entrant 3
past race info

maybe a template of some kind or just a form
