Creating an excel worksheet from MS Forms

Is it possible to create a new excel worksheet using Microsoft Forms based on a template I have created?
I would like a form that creates an excel spreadsheet and enters basic details into a template I have set up

This may help you. i used his video to create a chemical inventory form to be used with IPads.

This is from Kevin Stratvert, but there are several other videos that may help.
