Dlookup on Multiple Criteria

Hi guys,

I want to DLookup a value from one table to bring into a Select Query based on Multiple Criteria. Basically I want the Query to do the following

Where the value in:

[tblData_StaffDatabase]![SDB_EmployeeNumber]

matches:

[tblData_ChangeLog]![CL_EmployeeNumber]

AND

[tblData_ChangeLog]![CL_Week] = 26 **numerical value**

AND

[tblData_ChangeLog]![CL_Year] = "2011-2012"

then return:

[tblData_ChangeLog]![CL_OracleCCNumber]

Can anyone help?

How do I change the 26 to show an input box? So that I can determine a number?
