Double VLookup or Double Index/Match?

Hello Guys,

I am having trouble using Index/Match..I have looked through previous threads but still no luck

My Excel is set-up in this fashion

A B C D E F G H
09/03/2008
09/04/2008
09/05/2008
09/06/2008
09/03/2008

My data set that I want to grab data is in the form:

QTY
09/03/2008 A 100
09/04/2008 B 200
09/05/2008 C 300
09/06/2008 D 4000

What I want to do is if the Letter (A,B,C,D) matches and the date matches, return the QTY. I am doing this because there are multiple same date entries so a simple vlookup wont work.

Any help is appreciated.
