Enter a homemade form on to a second sheet and clear form

I'm trying to use a homemade form (Sheet 1- Weekly Data Entry) to enter data into another page (Sheet 2- Exploded...) The only data that needs to be entered on to (Sheet 2) is the total score on bottom right of form. Also, once the data is entered, I want there to be some indicator on the (sheet 1) that the data is entered. Can someone please help with this? Its very much appreciated.
