Fill out a form by what you enter in a cell

I don't know if this can be done, but I am hoping that maybe someone can come up with an idea. I have a log which looks something like this:
Approval # Date Ctrl # Status type event $available $applied
400057 3-1-11 215651 dc SPR event 100.00 100.00

What I need to do is that when the status column gets filled in with a code like dc or any other kind of closing code, that a disposition form (which is a seperate excel worksheet) gets filled out automatically with what is found on the log by looking at the approval number. I know how to do vlookups, but how do I get a new disposition sheet filled out each time? Below is an example of a dispostion form.

Date: Approval #: Ctrl #:

Close SPR
_____ (dc) Did not perform Reverse ___________________
_____ (tc) Completed
_____ (pc) Partially performed, Reverse _________________

Any suggestions? I thought about a macro, but I don't know anything about using macros in excel. Any help would be greatly appreciated.
