Form in need of Formulas

I have a self made form that needs to be finished with formulas to make it work the way I want. Should be easy to do for someone who knows formulas and button macros. Willing to pay a fee if needed. I can send you the form and explain what needs to be done via a phone call. Willing to re-structure form if you see an easier way to complete final solution. Thanks.
