Form not working please help

I have this code on a Submit button and the button is named SubmitButton3, after I press Submit the form disappears and no data is logged in my columns. Any Ideas?

My first thoughts are that "Date" is not a legal object name and "Name" is a confusing object name. Changing those might let the code do its thing.

PCRIDE, please use the CODE tags in the future when posting code. It makes code easier to read.

Anyway, regarding the issue..

When you define the variable NextEmptyRow, you're not assigning it to a specific sheet (like you're doing with every row below it). Try changing

Also, check your names for "Date" and "Name". Those are reserved words in VBA.
