Help with Suitable formula to fetch data from one continuous table to another table arranged in columns

Dear Friends,

I have a data table (its a long table some abstract given here), I have to fetch the Data given in the "DISP ST" Column in another table (To be solved table) against the Car+Shift on that particular date which is entered in a series of Columns

Appreciate your valuable advise to solve the problem
