How do I extract information from table to formulate an input (row & column)?

Hi there,

So I am trying to extract information from the below table to formulate an input based on the number of days by each column reference....

I would need the names to populate on multiple rows with the appropriate column reference and counts as shown below. Is this possible to do? This would create a series of data to then input into a program, but the refences need to show on multiple rows which makes it tricky.

I would use a 'tabular form' pivot table.
Then 'repeat all item labels'.

When I put this in a tabular pivot table, the information goes along the columns and I am not able to show the name and type in the rows. Are you able to assist?

Name should be in rows. Your extra columns on-site, off-site, etc should be in Values.
