Index / Consolodate

I have a sheet with the same name on multiple lines and then monthly sums. Example:

so i could Index - but that would required multiple tabs...

looking to index and consolidate matched names in A and consolidate the figures in B-F on a seperate tab for sheet from the data... so it's not index.
