INDEX MATCH IF, nothings working for me?! BrainFart I think

Can anyone solve this with a formula? I need to obtain each "DATA" instance in the "FORMULAS" column. Its currently just index match and move one down through counta, but as you can see by yellow its not working correctly. Any help is appreciated and I'm sure this is quite an easy solve for you geniuses!
