Index/match with 2 match criteria for column

Hi all,

It is possible to use and AND() to create and two criteria match statement or is there another way to do this. My formula is:

=INDEX('US001'!$B$7:$BA$200,MATCH($C4,'US001'!$A$7:$A$200,0),AND(MATCH(D$2,'US001'!$B$5:$BB$5,0),MATCH(D$3,'US001'!$B$6:$BB$6)))

Which is returning N/A....sample data:

I am wanting to return the value based on the year AND the month numbers.

...confirmed with CONTROL+SHIFT+ENTER.
