Linking Option Buttons and Combo Box

I am trying to build a survey in excel for over 20 people to take. I’ve developed the survey using 4 option buttons within a group box (all from the control toolbar) per question. There are 45 questions. I also have a tab for store all the responses so I can run reports from the single data tab.

Does anyone know if / how I can get a name selected from a combo box (also from the control toolbar) to store the responses to their option button selections to their name in the data tab. I would need to change each option button’s cell link for each different name.

I envision people able to open the excel file, choose their name from the list, take the survey and then save. Once everyone takes the survey if I go to the data tab then everyone’s responses are saved and I can run my reports.

Help would be greatly appreciated!
