Multiple CountIf for Excel 2003

Hi,
I'm trying to use multiple COUNTIF functions but am using Excel 2003, so cannot use COUNTIFS
In the following chart, I am trying to come up with a formula to be able to mark in column C if this is a new pet that we're seeing. Perhaps there is something easier than COUNTIF, but I am failing with everything I try.
