Mysterious Disappearing Conditional Formatting

So I saved a file that had this Conditional Formatting rule:

IF:

=D4<>'Prior Scorecard (Automated)'!D4

THEN: Bold and Underlined.

Worked wonderfully. Then I closed the file after saving. Opened it two seconds later and poof. All conditional formatting rules went bye bye for no apparent reason. Halp?

NOTE: Just tried this a second time. Set up the rule, saved, worked fine, then closed, re-opened, and rule is gone.
