Percentage calculation formula

I have a spreadsheet that works out a monthly percentage for commission payable based on some parameters. (download here: Incentive Plan Matrix.xlsx)

Two parameters need to be met, a revenue attainment and a lead delivery attainment, commission payable is dependant on percentages achieved for each of those parameters. I just want some help with the formula with the exact levels. ie 125% and 125%, 100% and 100%, 80% and 80%.

On the spreadsheet I have four tabs, two are the original tabs and the other two are the answer I'd like the cells to show. The cells I'd like the formulas for are highlighted in green.

The Matrix Answer tab: I need to formula for cells G7 and H8. I need them to match the monthly percentage in the table at the bottom of tab Sample Attainment Answer. (63% and 42%).

Any help would be great on this

Excel Facts

Whats the difference between CONCAT and CONCATENATE?
The newer CONCAT function can reference a range of cells. =CONCATENATE(A1,A2,A3,A4,A5) becomes =CONCAT(A1:A5)
