Potential vs Performance 9 grid plotting

Hi All!

I'm came across this talent review template which I felt was pretty useful and would like to adopt it into my talent review process. However, i'm trying to do up the 9 box grid but I can't seem to figure it out! Anybody out there who can possibly help or guide me on how to get this 9 box grid (potential vs performance grid) done with the auto-plotting with the filling up of the numbers?

I've modified the original with extra rows on the "Rating" sheet and formula in Rows 8&9 on the hidden "Office Use Only" sheet.
