Prefill Form With Pivot Table data

I have a data dump from our system at work that allows me to create a pivot table and see sales for the past 12 months.

I`d like to create a form that would show this monthly sales data for any specific customer, and allow the user to enter 2 additional pieces of data for any 1 product line (Q On-Hand, and Proposed Order).

Then, I`m assuming you submit the form, and all that data would become a record that the user could open at a later time when they were ready to enter the order into our system.

This way, when they open the record, they have the list of everything the customer carries, the Q of each item on-hand, the 12 month history, and their proposed order for that customer.

Can someone sort me out? Is this even possible? Am I going about this the wrong way?
