Problem with hyperlink and vlookup error "Cannot open the specified file".

Here is my formula it shows the referenced cell text and it is hyperlinked but will not open the file.(PDF) Hyperlink does work in the original cell but not when I try from another sheet in same workbook.
=HYPERLINK(VLOOKUP(P2&"|"&F5&"|"&G5&"|"&H5,HYPER!E2:F7,2,FALSE))
Any help would be appreciated.
