How do I create a pivot table report to transform data as follows?

Excel Facts

Format cells as date
Select range and press Ctrl+Shift+3 to format cells as date. (Shift 3 is the # sign which sort of looks like a small calendar).

Does anyone have an answer for this? I'd be really interested in figuring out how to transform data like this. I've got a huge amount of data that I'd like to transform, but I can't get it to work in a pivot table.

Please help, thanks.
