send Email depending two variables and containing a tkt number from another field

Right, this isn't the first time that i have tried to get this code, i know it's quite easy, but i just don't see it.

I have the column C that has the state of the ticket closed/open/error/MP, if this is "O", then i need to get an alert from column D which has the opening date in it, but the email must contain the ticket number from column A.

There must be two types of alert one when it is 5 days and another when it is 10 days over due.

If i could get anything basic i could adapt it the problem is that i can't see it.
