Spread revenue using start date & # of periods

I want to spread a revenue recognition forecast across months. I have the following fields to work with: contract #, total revenue, revenue start date MM/DD/YY, and number of periods that the revenue will recognize across. If I add columns for future months going out for 12 months, what formula can I use to reference the fields mentioned above so that the revenue recognition populates in the correct periods and at a monthly average amount?
