Survival Curve template

Does anyone know where I can get an excel template (2003) wrere I can add values an it calculates and draws a survival curve??

Yeah, seems not to be there.

Go to the Appendices in that paper. Appendix A is the formulas; Appendix B is the macro code.

Im no expert. I have copied the macro and the test data but not sure what to do with all the formulas
