To generate the label in PDFfrom excel file

dear all

I would like to thank all of you for all the support. I am stuck this file which is attached.

I want to generate the labels in pdf. when I click "Generate Label", it should generate the label with the length of 10 cm and height of 5 cm on pdf.

When I put on box number 1 out of 15, it should generate 15 labels. On box number, each labels should be generated as 1 out of 15, 2 out 15, like wise.
