UserForm search workbook

Does anyone have an example of a userform that is able to use two textbox values to search the workbook for the same values and pull the values in the same row into the userform.

Such as the two texbox values would be first name and last name.
The values it is pulling besides the names is sex, date of birth, class, and grade.

Any help would be nice. Thank you

On 2003 there's an option called 'form' in the 'data' menu.

If you select any cell within your data and activate this option, a form appears. There's a 'criteria' button on the form which may achieve what you're after.
