Userform to collect/send data

I have been assigned a project to collect bar code data and save it. I would like to create a userform to do so. I would like to use text boxes to collect the data after I scan the bar code. After scanning numerous bar codes, I would like to assign it a date and send it to Access.

I am a beginner here and would appreciate any help you all can give me.
