Vlookup with multiple searchfields

So I have 2 databases that I need to use to look up position titles for cellphone useage and am trying to figure out how look up a last name and if true proceed on to look up a first name and then return a value. To illustrate I have the following table to show employees who have cell phones:

My question is how do I do a vlookup with the lookup value of the last and first name on the first table, and a table array of the second table with a return value of column 3.

Any help would be appreciated.

Ctrl+S is used for Save. Ctrl+5 is used for Strikethrough. Why Ctrl+5? When you use hashmarks to count |||| is 4, strike through to mean 5.
Its dirty, but couldn't you just concatenate lastname and firstname in both tables and just do Vlookups from the concatenated fields?
