Why is my INDEX-MATCH formula not working?

I'm trying to test out an INDEX-MATCH combo formula, but for some reason, it's just not working and I got a #REF! error. I followed step-by-step the guides I've read online so I'm not sure where I went wrong?

Ok the formula itself seems to work now but the value still isn't correct. The formula should return "tom", but it returns "west" instead...

I found the solution... needed to reverse the Columns and Rows fields of the formula

That is correct. However, another way would be
Excel Formula:
=FILTER(FILTER(C9:E13,B9:B13="T1"),C8:E8="TEST2")
