checkboxes vs radio buttons

I am building a small form and want to use radio buttons to answer questions. How do I have a few buttons for different questions and make each question answered with a selection of buttons. When I put radio buttons on more than one question, they are all linked and only one radio button can be selected on the whole worksheet.

You need to group them with a "Frame" or "Group Box" (same toolbar).
