Convert invidivudal columns with values into two columns one date on value

Hello,

Trying to convert a sage 1000 budget table which has separate columns for month into the same format as the transaction table (one date column one value table). In order to compare the values side by side.

At the moment in time there is one year column and 12 month columns each with the values going down. I need to in power pivot or sql convert that into the column structure.

Any Ideas
