Could excel VBA have this item in user form?

Hi,

I'm wondering could VBA user form have the input box function as the picture show below?
My purpose is to make a user form with 2 parts which could submit the data into multiple rows.

example:

row 1 = form Part A + Part B1
row 2 = form Part A + Part B2
row 3 = form Part A + Part B3...etc

Each input have different rows in part B and might up to 50 rows.
Also, is it possible that not only to use this form as an input but also an record output/report as well?

Whilst you can use a listview, I'd look at using a listbox if you are all concerned about compatibility

Yes Kyle's suggestion is another good approach.
so you can make your UF look like this:
