create a questionaire

I'm trying to create a questionaire in Access..however I'm not quite sure how the structure should be...I have a table of 10 different questions with possible responses. Which are yes/no and N/A. But I want the 10 questions to appear on a form as question #1 - #10, all on the on form...as if you're looking at a sheet with check boxes next to the question. I don't want to have to scroll through each question...am I making sense? Does anyone have any suggestions at all on how to proceed??

I guess you'll want to track responder details too?

If so, my thoughts as follows.

2 Tables

tblResponder
responderID (Autonumber - PK)
RfirstName
RSurname

tblQuestions
AnswerID (autonumber PK)
responderID
Question1
Question2
...
Question10

All the Question filelds can have a valid entry of YES/NO/NA

How you set it up on the form could be just Labels containing the Questions & checkboxes / comboboxes to capture the response

To be honest... you needent have the responder table if you don;'t want it - just put the details on the Question table

Thanks for the response!!! The thought of using labels for the questions had crossed my mind a day or so ago...I'm not sure what happened to that idea though... :wink: it sort of just fell along the wayside...I'll give your suggestions a shot!!...
