Create New Row of Data in Excel

Hi All -

I'm trying to find a way to create new rows of data where there are multiple dates for different codes for the same employee. I've attached a spreadsheet of sample data.

Basically what I need to do is bring forth the records with earlier dates with the next date in sequence. Columns A-C is my raw data. Columns H-J is what I hope to achieve, creating the new rows highlighted in yellow.

Any ideas would be greatly appreciated.
