Excel duplicating worksheet in VBA and changes type to Workbook

Hello everyone,

I have come across with weird thing. For unknown reason excel duplicated one of my worksheets renamed it to from Name to Name1 and changed the worksheet Name type from Worksheet to Workbook. Do you have any idea why that happened?
All the code stored under affected module has landed to the one that was converted to a workbook.
The excel file is stored in Sharepoint and shared accross the team.
Recently I have added the ActiveX ComboBox to the affected worksheet, but I did not find if that could be the reason.

Excel Facts

Shade all formula cells
To shade all formula cells: Home, Find & Select, Formulas to select all formulas. Then apply a light fill color.
@Jeffrey Mahoney thank you for the response. It was the ActiveX objects that caused the issue. I decided to change the approach instead of creating fresh workbook as it was too much going on there already. And deleting those objects helped.
