Formatting pivot table columns

I have a pivot table I cannot get the columns formatted like i need them. I want them to look like the table highlighted in yellow. The one highlighted in red is the one I cannot figure out. What am i doing wrong.

Under the Design Tab to the very left of the tool bar select Report Layout.
(you need to be in the pivot to see the design Tab)
I believe the pivot you want to change is using Compact Form and that your other pivot is in Outline Form.
So see if Outline Form give you what you are after,
(My personal preference is Tabular Form)
